The third is a report with no photographs, which makes it impossible to confirm where the sample came from. The fourth is an undated or post-dated report, which raises the question of whether the material was disturbed before anyone knew what it was.
Sampling strategy on a loss
On an active claim you are balancing thoroughness against speed. The practical rule: sample every homogeneous material the mitigation scope will physically disturb, plus any material immediately adjacent that could be damaged during the work.
For a second-floor supply line failure, that usually means the ceiling texture below, the joint compound if walls are opening, the flooring if it is being pulled, and any duct wrap in the affected cavity. Four materials, one visit, one report.
It costs less to over-sample by one material than to send a sampler back. Multi-material scopes are quoted before collection so the number in the file is the number on the invoice.

The photograph standard
Every sample location gets a wide shot showing the room context and a close shot showing the sampled spot with an identifier visible. That pair of images is what lets a reviewer eighteen months later confirm the report describes the material that was actually removed.
Photograph before sampling as well as after. A pre-sample image of intact material is frequently the piece of evidence that resolves a dispute about whether the material was already damaged when the crew arrived.
Chain of custody, plainly explained
Chain of custody is a signed record of who held the sample from collection to analysis. It lists the sample identifiers, the collector, the date and time, the transfer to the laboratory, and the laboratory's acknowledgement of receipt.
It exists to prevent exactly one argument: that the sample analyzed was not the sample collected. In litigation and in subrogation, that argument is the first one raised against a report that lacks it.
Writing the scope so it maps to the report
Use the report's material descriptions verbatim in the scope. If the laboratory calls it 'white textured ceiling spray, none detected', your scope line should reference that material and that result.
Where a result is positive, the scope must show licensed abatement, not general demolition. Where a result is negative, the scope can proceed as ordinary construction demolition. Making that logic visible on the page is what turns a reviewer's job from investigation into approval.
